How to Make a Referral to Live at Supportive Housing for Young Mothers

Step 1
Pregnant women and mothers aged 16-21 interested in living at SHYM should contact their Income Assistance Worker to inquire about the program, and learn how to apply.

What Happens next?

This initial telephone conversation with the Family Support Worker is to outline SHYM’s program and to gather information from potential applicants to determine eligibility.

Following this step, a meeting can be scheduled to discuss in more detail the SHYM program, and to determine if it is the right program for the applicant.

If so, a tour of SHYM will be scheduled.  If an applicant then wishes to live at SHYM, pending approval the appropriate paperwork will be completed, and a move in plan established.

After this, regular meetings with the Family Support Worker will be arranged. These meetings are to get to know one another, plan goals and put programs in place that will best suit the tenant.
