1. Call your Income Assistance worker to tell them you want to live at SHYM.
2. Call 465-2212 and ask to meet with our Family Support Worker.

2. Print off our on-line application form and fill it out.  Then call 465-2212 to ask to meet with our Family Support Worker.  Bring the form with you to the meeting.
